THANK YOU FOR SUBSCRIBING

Jamie Thomas, CEO and Co-founder, Cognito FormsCognito Forms makes an impact by empowering healthcare providers with a secure, compliant solution to create powerful workflow-enabled forms that streamline practice management and patient onboarding. To deliver a highly distinguished solution, it focuses on three core aspects; provision of powerful forms, ease and accessibility, and affordability.
These customizable forms are designed to streamline and accelerate manual, paper-heavy data collection processes in organizations. In just minutes, these care providers can use these forms to accept payments, collect data, and create powerful workflows to automate processes and work more efficiently.
Data such as a patient's age or date of birth may seem easy enough to collect, but when there is an overwhelming influx of patients, speed and accessibility become crucial. Cognito Forms’ solution includes features for repetitive data like medications and medical history and easy-to-use tools for calculating age and other processes to simplify workflows. The solution can be integrated with EMR systems to facilitate seamless data entry and retrieval.
The ease with which anyone can use the form templates without needing a developer or being overly tech-savvy is also critical for many clients. Cognito Forms provides high-quality solutions at affordable prices and no contract, benefitting small organizations that cannot afford expensive solutions while helping them meet their capability, performance, and security requirements.
“We are helping organizations fill gaps in their security and information privacy operations as part of their digital transformation journey while allowing them peace of mind to focus on enhancing care delivery and elevating patient experience,” says Jamie Thomas, CEO and co-founder of Cognito Forms.
When patients are admitted to a hospital, they must fill out forms, but doing this for every visit is redundant. Cognito Forms creates a process for patient onboarding that is intuitive and practical. The health organization can sign up for any of Cognito Forms’ plans without any complex registration process or serious developer input. All they need to do is choose any patient assessment template or customize one to suit their operation. These templates can be used for bill payments, patient surveys, prescription refill requests, and other functions.
Thomas says, "We go the extra mile to help all our customers, regardless of their plan level, understand how to use and adapt the product to their needs. This interaction helps us understand how to deliver better services and tweak our products to suit the customers."![]()
We're helping organizations fill gaps in their security and information privacy operations as part of their digital transformation journey while allowing them peace of mind to focus on enhancing care delivery and elevating patient experience
In one instance, Cognito Forms helped an emergency operations center in Chambers County, Texas, set up forms to accelerate their service delivery. This healthcare organization wanted to administer 400 doses of the COVID-19 vaccine in rural areas but needed to ensure that the right people reached the right location at the right time. They used a template by Cognito Forms to streamline registrations, including the screening information necessary to keep these vaccine recipients safe. The data was integrated directly with their EMR, which directly updated the immunization registry as required by law. In just seven minutes after opening registration, 400 people registered for their vaccination.
Cognito Forms' top-notch customer service is one of the best aspects of their team. With automation, accessibility, and affordability, it is doing its part to expedite and enhance healthcare delivery.

The significance of the EU-US Data Privacy Framework is as follows:
The EU-US Data Privacy Framework is a recently established agreement between the European Union and the United States. It provides a legal foundation for the transfer of personal data from the EU to the US. This framework was developed in response to the Schrems II ruling, which invalidated the previous EU-US Privacy Shield Framework.
Although Cognito Forms has been a participant in the EU-US Privacy Shield Framework since 2018, the Schrems II ruling found that this framework did not offer sufficient protection for the personal data of EU citizens. The framework was criticized for allowing US intelligence agencies to access EU data without adequate safeguards.
To address the concerns raised in Schrems II, the EU-US Data Privacy Framework introduces various new safeguards, including:
• Restricting access to EU data by US intelligence services to what is necessary and proportionate to protect national security.
• Establishing a new Data Protection Review Court to investigate and resolve complaints regarding access to EU data by US intelligence services.
The EU-US Data Privacy Framework represents a significant step forward in ensuring the secure transfer of personal data between the EU and the US. It is of great importance for businesses that operate in both regions and provides EU citizens with the assurance that their data is protected.
As part of our participation in the EU-US Data Privacy Framework, we have updated our privacy policy to replace references to the EU-US Data Privacy Shield with the EU-US Data Privacy Framework. Even after this change, we continue to advise users to sign our Data Processing Addendum, which includes the latest Standard Contractual Clauses (SCCs) and the UK extension in compliance with GDPR and UK GDPR regulations.
Changes to US state-specific data privacy laws:
The privacy landscape within the US is continually evolving, and we are committed to staying informed about the latest regulations and best practices. We prioritize "privacy by design" and maintain transparency with our customers regarding data collection and usage.
After conducting an internal review, we are pleased to confirm our compliance with all current and forthcoming state-specific data privacy laws, including but not limited to the California Privacy Rights Act, Colorado Privacy Act, Connecticut Data Privacy Act, Delaware Personal Data Privacy Act, Indiana Consumer Data Protection Act, Iowa Consumer Data Protection Act, Montana Consumer Data Privacy Act, Oregon Consumer Privacy Act, Tennessee Information Protection Act, Texas Data Privacy and Security Act, Utah Consumer Privacy Act, and Virginia Consumer Data Protection Act. Our privacy policy has been updated to reflect these alignments.
What does this mean for you?
As a Cognito Forms customer, you can have full confidence that your data is secure, regardless of your location. We are dedicated to upholding your privacy and strictly adhering to all relevant data privacy legislation.
