The software provides staff with customized workflows, plan of care, promotes seamless communication among the care team members, and permits security driven access to different suppliers and team members to accomplish individual strategies. FidelityEHR’s embedded dashboards and rich, and flexible reporting, enable data-based decision-making and ongoing evaluation of success and barriers. The software utilizes embedded HIE technology to mobilize, transmit and consume health record information electronically across data systems to support client care coordination. Access to the Medicaid Export and EDI submission features are permissions-driven and established by each user role. “With this platform, family members or patients can login to their mobile phones to get the required health information, and receive text messages about matters that need immediate response. Further, the FidelityEHR team, is the first care coordination software provider to participate in a rigorous random control trial. Early results of the trial are promising and suggest that the software can not only improve care coordination staff efficiency and effectiveness, but that the software can increase fidelity to evidence based practice for care coordination, a model that that has been demonstrated to improve client outcomes.
