Matt Schubert, Chief Strategist, FidelityEHRHaving extensive experience in serving the health and human service industry for years, Schubert says, “Data Sharing Policy issues and interoperability with legacy systems are considered to be the major roadblocks in implementing HIEs.” With the protection of patient data a pressing concern, healthcare organizations need to focus on finding appropriate ways to share information securely across different systems and boundaries. FidelityEHR delivers a secure and user-friendly EHR platform that supports the evidence-based principles of Wraparound and integrates research-based clinical care coordination.
The FidelityEHR software equips care teams to collaborate, coordinate and tailor techniques to each customer’s needs. Designed by taking into account the Meaningful Use 2.0 Electronic Health Record (EHR) requirements, FidelityEHR enables interoperability with other EMR and EHR systems while improving the ability of systems to more efficiently incorporate a team-based approach to care coordination. Including the advancement and management of plans of care, crisis plans, progress notes, incident reports and complete youth, family, and team information.

With this platform, we’ve created care coordination software that staff love because it makes their work easier and more efficient
The software provides staff with customized workflows, plan of care, promotes seamless communication among the care team members, and permits security driven access to different suppliers and team members to accomplish individual strategies. FidelityEHR’s embedded dashboards and rich, and flexible reporting, enable data-based decision-making and ongoing evaluation of success and barriers. Unlike traditional electronic health records, FidelityEHR provides the right information to help each team member make the best decisions possible to drive positive outcomes for patients.
The software utilizes embedded HIE technology to mobilize, transmit and consume health record information electronically across data systems to support client care coordination. FidelityEHR permits the administrator to exchange critical data between sources in a timely manner, through a computerized exchange process or in real time, depending on the data needs. Access to the Medicaid Export and EDI submission features are permissions-driven and established by each user role. “With this platform, family members or patients can login to their mobile phones to get the required health information, and receive text messages about matters that need immediate response. The staff can also access information across different devices,” explains Schubert.
Further, the FidelityEHR team, is the first care coordination software provider to participate in a rigorous random control trial. Early results of the trial are promising and suggest that the software can not only improve care coordination staff efficiency and effectiveness, but that the software can increase fidelity to evidence based practice for care coordination, a model that that has been demonstrated to improve client outcomes.
