A leading Wisconsin-based health plan, serving 18 hospitals and over 130 outpatient sites, partnered with Kbrax to overcome persistent challenges in post-acute care management. Prior to the engagement, the organization faced low primary care provider (PCP) follow-up rates, average wait times of 34 days for PCP appointments, high rates of emergency department (ED) utilization, and preventable readmissions. These issues placed immense strain on care teams and drove up costs across the network.

PCP follow-up rates for high-risk members increased to 56 percent. Follow-up wait times dropped to just 10 days post-acute event.
