At its core, interoperability is best described as the ability of diverse healthcare information systems to communicate, exchange data, and use it coherently and meaningfully. The concept is basic yet crucial for various healthcare activities, from coordinating patient care by diverse providers to ensuring that emergency responders have immediate access to critical patient information.

Despite this critical need for true interoperability, several significant difficulties remain. Chief among these is the relative need for standardized data formats and protocols. Health-related data may come in many forms, including but not limited to EHR data, laboratory results, imaging, and patient-generated data. All these systems could utilize different standards or no standards at all, thus leading to a patchwork of various data formats that complicates integration and data sharing.
The problem is partly due to the proprietary nature of many healthcare systems. While vendors are keen to ensure optimal functionality within their ecosystem, interoperability with other systems may be a lower priority. This perpetuates silos of information that are difficult to bridge, leading to inefficiency and possible risk to patient care. Volume adds to the problem because so much data is generated, and patent privacy needs protection. It should be accessible health data, yet it must be locked down with a sensitive balance of protection standards regarding data, including those guided by regulations such as HIPAA in the United States.
This is compounded by the fact that healthcare providers are at different levels of technological sophistication. Smaller practices or those in underserved areas may not have the money to invest in state-of-the-art interoperable systems. This contributes to vast chasms in the quality and continuity of care-sharing of information during patient transfers and emergencies.
Such barriers are being overcome by the development of common standards and frameworks, such as the Fast Healthcare Interoperability Resources standard, which builds a common way to exchange healthcare information. Secondly, greater data sharing is being promoted through legislation, such as the 21st Century Cures Act in the United States, including efforts to reduce information blocking.
This is promising, but only part of what will be required. True interoperability will depend on sustained collaboration among the stakeholder groups responsible for providing health services, developing enabling technologies, and setting policy guidelines. It also will require a deep-seated cultural shift to vest interest in sharing and transparency while providing adequate security measures for patient information.
Fully interoperable healthcare is a very ambitious undertaking. At the same time, it represents a multidimensional challenge that requires the simultaneous application of technological innovation, standardization, and adherence to regulations. As we progress, harmonizing the systems and active collaboration will form the crux for realizing the full potential of seamless, patient-centered care.
