The LeoMed platform’s open-toolbox approach allows healthcare providers and physicians to design and deploy automated medical-data collection and analysis tools through connected devices and intelligent communication pathways. To get a better picture of how these features benefit healthcare organizations, one can look at a recent client engagement of LeoMed. In this instance, the Canadian Centre Hospitalier de l’Université de Montréal (CHUM) struggled to accurately triage and determine the best-suited care delivery trajectory. Initially, CHUM required its nurses to make close to 30 calls for the timely preparation of patients before their scheduled surgeries. Despite the process, CHUM witnessed many last-minute cancellations on the day of surgery, with around 11 percent of patients not being prepared for the procedure. Even in the case of successful surgeries, CHUM witnessed certain workflow inefficiencies.

For instance, the healthcare organization had to spend considerable time following up with patients and guiding them through the post-operative phase, which still led to 10.5 percent of post-surgery patient readmissions. With the LeoMed platform, CHUM could share surgery-related videos and pre-surgery notifications with patients to better prepare them, reinvigorating their care delivery model. The collaboration with LeoMed resulted in better surgery day preparedness among CHUM’s patients. Alternatively, for post-operative guidance, CHUM gained the ability to send automated follow-up questionnaires and videos for pain and side-effect management through LeoMed. This resulted in a drastic reduction of CHUM’s total readmission rate.
