No patient stays healthy or sick forever; people’s health changes over time. Understanding this, care providers in the current patient-centric healthcare system are laying down a care trajectory that aligns with patients’ specific conditions and needs. However, when attending to hundreds of patients every day, it becomes challenging to have a finger on the pulse of their patients’ varying health conditions. Helping care providers and physicians in this regard is LeoMed.
“Our eponymous LeoMed platform orchestrates medical care trajectories and empowers physicians, patients, and all other actors within the care continuum to create a seamless care trajectory by assessing patient’s health condition in real-time,” says Dr. Joseph Aubut, founder and CEO of LeoMed. Born of Dr. Aubut’s personal experience as both a patient going through different health concerns and a physician trying to deliver effective care, the LeoMed platform aims to make healthcare more accessible and efficient in the era of connected, personalized, and remote care delivery.
The LeoMed platform’s open-toolbox approach allows healthcare providers and physicians to design and deploy automated medical-data collection and analysis tools through connected devices and intelligent communication pathways. Caregivers can develop three distinct modules—LeoMed Flow, Monitor, and Connect—that optimize patient follow-up and monitoring. The LeoMed platform allows users to create complex healthcare protocols, including care delivery tasks, treatment questionnaires, critical communications, and role management. Going a step further, the platform also positively impacts the quality and accessibility of care by offering tools for remote patient monitoring.
The platform allows a wide range of patient care situations to be addressed. With LeoMed, care providers can use templated or customized care protocols effortlessly and intuitively. The templates are primed for logic-based branching, which considers a patient’s response to a particular query before configuring the form to ask more relevant questions.
This makes medical triaging more accurate, data-driven, and effective. On the other hand, healthcare administrators can use LeoMed to monitor and assign tasks to different care providers, facilitating the creation of efficient care management pathways across multiple actors with different responsibilities and perspectives. Besides, LeoMed’s use of new-age communication and collaboration channels allows care providers to integrate patients’ family members and create a truly collaborative care delivery model.
To get a better picture of how these features benefit healthcare organizations, one can look at a recent client engagement of LeoMed. In this instance, the Canadian Centre Hospitalier de l’Université de Montréal (CHUM) struggled to accurately triage and determine the best-suited care delivery trajectory. Initially, CHUM required its nurses to make close to 30 calls for the timely preparation of patients before their scheduled surgeries. Despite the process, CHUM witnessed many last-minute cancellations on the day of surgery, with around 11 percent of patients not being prepared for the procedure. Even in the case of successful surgeries, CHUM witnessed certain workflow inefficiencies.

For instance, the healthcare organization had to spend considerable time following up with patients and guiding them through the post-operative phase, which still led to 10.5 percent of post-surgery patient readmissions. With the LeoMed platform, CHUM could share surgery-related videos and pre-surgery notifications with patients to better prepare them, reinvigorating their care delivery model. The collaboration with LeoMed resulted in better surgery day preparedness among CHUM’s patients. Alternatively, for post-operative guidance, CHUM gained the ability to send automated follow-up questionnaires and videos for pain and side-effect management through LeoMed. This resulted in a drastic reduction of CHUM’s total readmission rate.
Inspired by many similar success stories and its early success in Canada, LeoMed is poised to bring its innovative platform to more care providers and physicians worldwide. LeoMed is also planning to enhance the platform by leveraging the newest technological innovations, keen to redefine the care delivery paradigm.
