True-to-Life Simulation Training Model for Pelvic Surgeries

The current standard for pelvic surgery training and instruction primarily relies on live surgery or the use of training cadavers. Although accompanying and assisting proctoring surgeons provides resident surgeons exposure to live surgeries, the inexperience of trainees might cause problems with patient care. While training on cadavers is an alternative, they can be expensive and have short shelf lives if not properly embalmed. To address these shortcomings, simulation training is becoming the standard for teaching gynecological procedures to residents.

The Miya Model consists of a pelvic frame and multiple replaceable anatomic cartridges built to accommodate various specific surgical procedures. It incorporates lifelike skin and organs, realistic cutting and puncturing tensions, and palpable surgical landmarks, which simulate actual surgical experiences. The pressurized vascular system, with bleeding action and an inflatable bladder that leaks water if damaged, enables the resident physicians and trainees to improve their technique and impart greater accuracy during live surgeries. The training model creates a realistic surgical experience where surgeons can clamp, cut, and tie pedicles and feel the “pop” when passing needles through surgical planes without patient risk.

The capabilities of the Miya Model enable Miyazaki Enterprises’ clients to create a safe educational assessment ecosystem for their residents. Generally, there is no way to determine how competent a resident is in their basic skills until they have been applied in the operating room. This is not the safest way to assess residents, however, with the Miya Model, residents and practitioners can assess their skills and practice what they have learned in a classroom before performing surgery on a live patient.

The company has developed and published an app to accompany the Miya Model, which provides an information portfolio to communicate the best practices to the residents. The globally accessible and free Miya app, or the Miyazaki app, can be downloaded on Apple and Android platforms to access videos explaining the step-by-step process of performing a vaginal hysterectomy, retropubic slings, transobturator slings, and more. The app also contains assessment questions regarding the procedures and an evaluation of the 3D Miya Model to help the company improve the quality of pelvic surgery training.
