Patient engagement can be improved by enabling home health agencies to scale up telephonic patient communication.
FREMONT, CA: Home health care is gaining traction as it is more cost-effective than the hospital and home care nursing. To provide patients with improved experiences in terms of home health care, CipherHealth, a patient engagement software company, has collaborated with Medline, a global healthcare company.
The collaboration will mainly focus on expanding CipherHealth's episodic engagement program—CipherOutreach—to enable the home health agencies to scale up telephonic patient communication. This has proven effective when deployed to engage patients between home visits by identifying and helping to prevent potential adverse clinical events, addressing patient experience concerns, and confirming upcoming home visits.
