FREMONT, CA: Operating rooms are the primary energy consumers and waste generators among their components. Operating rooms can consume up to six times the energy of the rest of the hospital and generate more than half of the waste. Healthcare organizations must address this issue to reduce greenhouse gas emissions and protect communities from climate-related threats.
Surgeons have a unique opportunity to rise to the position of leader. The operating room is the most responsible for waste generation. As a result, surgeons bear a large portion of the responsibility for reducing their carbon footprint.

Their participation can range from local green initiatives to developing national standards and policies that promote sustainable approaches to healthcare delivery. The enthusiasm for this cause is palpable, with individuals eager to launch similar initiatives within their organizations. Surgeons can initiate change by taking small steps such as proper waste disposal and wider adoption of recycling programs. Waste production can be reduced by collectively reevaluating usage habits and reducing emissions.
Sustainability extends beyond waste reduction and recycling. Researchers divided sustainability strategies into five categories: refuse, reduce, reuse, repurpose, and recycle. Quality improvement initiatives that reduced the operating room's environmental impact and lowered costs were identified.
Other cost-cutting strategies included turning off equipment and lights overnight and reducing the frequency with which non-contaminated anesthetic equipment was cleaned. Surgeons frequently use prepackaged sets of gowns, gloves, and surgical instruments. Unnecessary disposable items were removed from these sets to promote greener practices.
The amount of discarded waste, particularly disposables and plastics, is alarming at the end of each day and each surgical case. Furthermore, the researchers identified gaps in the existing literature that could inspire future research. For example, anesthetic gases account for roughly half of all greenhouse gas emissions from operating rooms. Evaluating interventions that target these and other sources of emissions could yield significant benefits.
The current framework can help surgeons introduce these interventions and investigate their impact. Implementing efficient technologies such as low-flow anesthesia machines, LED lighting systems, and greener practices such as waste segregation and recycling may be required. Changing to reusable linens and instruments in the operating room can reduce greenhouse gas emissions significantly.
The role of the healthcare sector in emissions is becoming more widely recognized on a national scale. Experts in this field are required to develop strategies for reducing emissions across hospitals and the healthcare system as a whole. In the future, every operating room will have a dedicated green operating room team comprised of sustainability-focused surgeons, nurses, anesthesiologists, supply chain experts, environmental services personnel, and hospital management. Collaboration to move towards greater sustainability while reducing carbon footprint would ensure that all stakeholders actively participate in decision-making, allowing hospitals to implement sustainable practices in the near future.
