A hospital clinician has just informed a patient that he has been diagnosed with a critical ailment. As part of the treatment plan the hospital provides an app called Plan-it Med which plays an integral role in the patient’s initial understanding of the disease. The application assists in the creation of new behavioral health patterns that will allow the patient to adhere to the new treatment regimen and the clinician to track his behavior.

Meanwhile, another patient is diagnosed with a disease that exhibits high rates of readmission. Over the course of time Play-It Health had provided the clinicians concerned with the ability to track the patient’s daily weights, medication, and adherence to recommended diet, eventually reducing readmission rates.

Such scenes are common for clinicians and patients leveraging the Play-It Health solution to improve adherence to healthy medical regimens. “Our intent is to use the latest in information technology to give patients, their friends and family, and clinicians peace of mind and the freedom to live their lives,” says Kimberly Gandy, the founder and CEO of Play- It Health. The company delivers a unique set of tools and resources to get patients to adhere to the recommended timing, dosage, and frequency of medication intake.

Their flagship product, Plan-it Med, is a mobile application which is the user-facing component of an agile platform that has backend customization, personalization, reporting, and analytics. Play-It’s system aggregates data from multiple critical elements—patient engagement, appointment attendance, linked device usage, nutritional regimen adherence, and medication usage— and feeds it into their unique scoring engine, PALM. The engine interfaces with the existing clinical regimen and tracks and incentivizes patients to enforce adherence to the prescribed health regimen.

“The PALM score is based on a broad set of activities that lead to overall better health outcomes: educational pearls (P), appointments (A), linked device data (L), and medications (M),” informs Kimberly. The PALM score not only serves as the ultimate communication tool for clinicians, patients, and third parties but is also a tool for stratifying patients and allocating resources. Based on the PALM score, patients can be classified into those that are in need of more supervision and those in the danger zone who require immediate intervention. Play-It Health also offers reporting and analytics tools that allow clinicians to track when patients fail to adhere to their medication course and serve as a digital coach that encourages patients to stay on track, allowing them to launch digital interventions when patients turn to non-adherence. Through these solutions, Play-It Health is able to provide low-cost and continuous access to care and clinicians.

Recently validated by the American Heart Association in its Innovation Showcase at the AHA’s annual meeting, the firm aims to launch its lined-up work in heart failure over the coming months with the idea that the solution is applicable to all chronic care populations within a hospital. Play-It Health is also rolling in speciality pharmacies and forming strategic partnerships throughout the healthcare spectrum to deliver a better engagement solution for patients.
