Its platform is a clinically proven remote rehab and personalized patient care solution designed to improve recovery and long-term survival rates and enhance the quality of life.

For in-patient settings, RecoveryPlus.health’s application connects patients with a well-versed and licensed EP who leverages EHR technology, Zoom, or other technology solutions to conduct one-on-one evaluations and formulate data-driven and tailored rehab strategies.

This holistically connected approach to care enables a seamless transition for post-acute patients from a dedicated medical facility to a customized home-based program, increasing ongoing treatment compliance while directly addressing risk factors.

From a preventative and proactive care standpoint as well, RecoveryPlus.health tailors exercise plans and care management strategies to empower patients with control of their heart health and wellness and prevent further complications from the comfort of their homes. Alternatively, for direct patients, its AI engine evaluates their historical data to provide personalized recommendations to the medical staff that validates and advises changes.

Initially, the patients at this facility spent an average of three months before discharge, and the client wanted to integrate dedicated cardiac rehab programs with their existing services. For this project, RecoveryPlus.health trained their staff and educated patients on the benefits of cardiac rehab. Simultaneously, a medical team member visited the Center regularly to evaluate patients and perform impactful cardiac rehab programs.
