As the Vice President of Operations at Hartford HealthCare, what are some of the roles and responsibilities you handle daily, and what was the journey like before you took up the role?

Throughout my many years in the military, I have held various positions that have shaped my experience. These include serving as a trauma medic, a flight nurse, a medical crew director, and a flight surgeon in the military. I have also been deployed multiple times. During that time, I learned valuable leadership and teamwork lessons, which have profoundly impacted me.

After several years in the military, I decided to work in the civilian sector, aiming to find a profession that embodies the key components of leadership and team spirit, with my healthcare background making the transition smoother. Consequently, my first job in the civilian world was as a phlebotomist in a hospital. Subsequently, I pursued a career as a nurse and eventually became an Advanced Practice Registered Nurse (APRN) specializing in family practice. Further down the line, I focused on infectious diseases epidemiology and American infectious disease under the guidance of Ulysses, the current Chief of Infectious Diseases at Hartford HealthCare.

Over the past two years, I managed Hartford HealthCare's response to the COVID-19 pandemic, working diligently to ensure efficient vaccine distribution within our community. Despite the challenging circumstances, we also identified opportunities to reduce hospital-acquired infections, albeit when adding another initiative seemed overwhelming. Nevertheless, we successfully elevated the mindset and changed the culture surrounding patient care from a safety perspective.

Following this accomplishment, the conversation shifted towards the operational aspects of managing COVID-19, which involved project management and implementing necessary changes to ensure safety and sustain the institution's viability. Communication was crucial in engaging our patrons and community members as partners in this process. Consequently, transitioning into an operational role seemed like a natural progression. I assumed this role a few months ago.

On a day-to-day basis, my primary focus is supporting individuals and departments within the institution, enabling their success within existing processes. This involves collaborating with various departments, such as Food and Nutrition, Dietary EDS, and infection prevention, to ensure they have the necessary infrastructure and processes. We evaluate suitable properties to facilitate our colleagues' and cohorts' successful delivery of care. As a leader, my responsibility is to ensure that people have the resources they need to thrive within the institution.

Considering the impact of the pandemic on creating a safe environment, what are some of the challenges you and your peers are currently experiencing?

In the immediate aftermath of the pandemic, one of the most challenging aspects has been the recovery process. The ever-changing nature of the information surrounding COVID-19 necessitated a dynamic approach that relied on constantly adapting to new data. Consequently, it was crucial to acknowledge the inherent uncertainty and be accountable for the decisions made during that time. However, as the environment has become more stable and we now have the ability to gather and rely on long-term information, we can implement programs with a greater sense of confidence. This transition to a more stable phase allows us to extend the efforts put in place during the recovery phase.

Undoubtedly, one of the most significant challenges lies in the burnout experienced by healthcare professionals. The impact of COVID-19 on mental health within the healthcare sector has been profound. We are witnessing a substantial decrease in individuals applying to and choosing careers in certain medical fields, such as nursing. This shortage in nursing personnel is expected to persist for the foreseeable future. Additionally, some individuals continue to grapple with the psychological aftermath of managing a disease for which we initially lacked adequate answers. The experience of watching patients succumb to a respiratory disease without having a definitive solution has been humbling and emotionally challenging. Similar to how we address PTSD in the military, we must acknowledge healthcare professionals' ongoing struggles in coping with the disease's management.

Furthermore, the economic climate poses considerable difficulty for healthcare institutions. Coupled with this is the need to strategically position ourselves differently. For instance, recent reports indicate a concerning shift in the production of personal protective equipment (PPE), with a significant percentage moving offshore. This raises questions about our ability to ensure a stable supply of essential items and avoid exorbitant prices during negotiations for critical resources like masks. Additionally, we must address concerns related to engineering aspects, such as air movement, to safeguard our institutions effectively.

Navigating these challenges while maintaining the delivery of high-quality care to all patients is an immense task. Undoubtedly, this period presents challenges for leaders in the healthcare industry. Striking the delicate balance between growth, innovation, and the well-being of our healthcare professionals requires careful consideration and a deep appreciation for what they have endured.

The healthcare workforce faced many challenges during the pandemic. To what extent have these conditions improved, and how do such improvements, or the lack thereof, impact infection control measures on a broader scale?

The healthcare profession entails immense challenges, and those who excel in it tend to be high achievers. However, being in a position where formulating a plan to save someone's life becomes difficult can be exceptionally demanding. Thankfully, the management of COVID-19 has evolved significantly, leading to positive outcomes for patients, even those with underlying health conditions. This has undoubtedly alleviated some of the pressure on healthcare professionals.

In terms of infection prevention, throughout the management of COVID-19, a clear message has been consistently emphasized, particularly within our healthcare system at Hartford HealthCare. This pandemic carries substantial mortality risks, underscoring the need for practices aligned with infection prevention measures to ensure our safety as much as possible. Consequently, hygiene scores have significantly increased during the peak of COVID-19 and have remained consistently high. Compliance with personal protective equipment (PPE) and relevant protocols has improved exponentially and maintains high levels.

While certain aspects have remained consistent, the mindset has shifted. Previously, there may have been a sense of frustration stemming from unsuccessful efforts to manage respiratory diseases in younger individuals, which defied existing epidemiological expectations. However, the current outlook has changed due to the availability of more effective therapeutics, advancements in virus understanding and mutation, and the introduction of vaccines. This shift in mindset has fostered a greater sense of hope among healthcare professionals.

Could you provide some insight into the best practices you have implemented to address staff shortages and enhance their preparedness in infection control? Considering that the ultimate impact of these practices directly affects the patients, have you observed significant outcomes due to these measures?

We have implemented several key practices to address areas of weakness and enhance our preparedness for infection control. Firstly, we identified the weakest points in our processes, particularly those influenced by variables beyond our immediate control. One such area was the availability of personal protective equipment (PPE), including masks and gowns. We revamped our storage and inventory management systems to address this, acquiring a large warehouse with an advanced logistics system. This investment ensures a responsible and continuous supply of PPE to meet the needs of our community.

Additionally, we have developed protocols that acknowledge the potential for future pandemics. Recognizing that the magnitude of COVID-19 has been unprecedented in recent history, we understand that globalization and changing climate patterns may lead to similar events in our lifetime. As a result, we engage in discussions and strategic planning for future outbreaks, ensuring that we are prepared to effectively manage them.

A crucial aspect of our approach is building robust processes rather than relying solely on individuals. For instance, when transitioning leadership roles in infection prevention, we have established best practices that facilitate a seamless transfer of responsibilities. This continuity ensures that our efforts in infection control remain strong, regardless of personnel changes.

Furthermore, we have embraced collaboration across different health systems. We can share insights and learn from one another by fostering better communication and partnerships with organizations like Hartford and Trinity. This includes engaging with health systems outside our state, allowing us to gain valuable knowledge and promptly implement proactive measures to detect and respond to emerging diseases or shifts in epidemiology.

What trends do you foresee considering the future of infection control over the next 12 to 18 months? Specifically, do you anticipate any changes in technology adoption or the emergence of new technologies? Are there any other best practices that will be crucial during this timeframe?

One of the most significant lessons we've learned from COVID-19 is the importance of community education. We dedicated substantial resources and time during the pandemic to ensure that people are well-informed about the necessary actions to stay safe. Moving forward, there will be a focus on improving communication with individuals in the community and providing them with clear guidelines for their safety when they visit healthcare facilities.

Moreover, infection prevention offers an epidemiological approach that can be further enhanced through advanced data analysis. Artificial Intelligence (AI) has made significant advancements in various industries, and I believe we can leverage AI to better assess risk factors and predict infections. By utilizing algorithms and criteria based on individual health elements, we can identify patients with a higher risk of specific infections, such as CLABSI or surgical site infections. Implementing these algorithms can significantly improve patient safety post-procedure.

While developing technology tailored specifically for healthcare can be challenging, we must explore models prioritizing prevention rather than reactive control. Machine learning and AI are likely to play a significant role in infection prevention within the healthcare industry, and we are already witnessing promising advancements in this field. Several companies are actively working on solutions in this area, and it will be interesting to see their progress in the coming months.

What advice would you offer to your peers and aspiring professionals in infection control? How would you suggest they navigate the challenges while staying on top of emerging trends?

Staying ahead in infection control requires a mindset of continuous learning and improvement. The ever-changing nature of information means that what we know today may differ from what we know tomorrow. Building a successful system should not lead to complacency but rather serve as an opportunity for further enhancement. As a leader or aspiring leader, investing in personal growth through education and nurturing diverse relationships is crucial. Building connections with individuals in various fields, such as technology innovators and epidemiologists from different health systems and countries, can provide valuable insights and fresh perspectives. Also, fostering a mentorship culture is essential for the sustainability of these specialized fields. By guiding and supporting the next generation, we ensure the preservation and growth of expertise in healthcare.
